Job Responsibilities may include but are not limited to the following:
Serves as the point-of-contact and provides technical assistance to local organizations within assigned region that are receiving KDHE grants, such as Chronic Disease Risk Reduction (CDRR). Conducts bi-monthly progress meetings, site visits, and provides grantees with resources and pertinent information to support community initiatives. Ensures grantee accountability, quality progress reporting and alignment of approved work plans. Participates in CDRR application process, technical reviews, and supports Community Health Promotion program goals.
Assists communities with identifying and leveraging resources to advance community health promotion work, build capacity and encourage the development of local coalitions for the prevention of chronic disease.
Works with supervisor and state-level chronic disease prevention staff to identify state resources (organizations, universities, funding, etc.) that are available to assist the local coalitions.
Assists with promotion of Seniors Farmers Market Nutrition Program in assigned region. Serves as the lead on special topics as needed and assigned by supervisor.
