Position: Outpatient Therapist / ACT Team Leader

Location: Colby, Kansas
Pay: $70,452 per year | $325 sign-on bonus
Job Type: Full-Time, In-Person
Schedule: 8am-5pm M-F (on-call rotation)

Why You Should Apply:

High Plains Mental Health Center is excited to announce we are launching our first Assertive Community Treatment (ACT) team in our Colby branch office, and we’re looking for a passionate therapist to lead the way!

The ACT model is a specialized, evidence-based approach to mental health care that goes beyond the traditional office setting. As the ACT Team Leader, you’ll guide a multidisciplinary team that meets clients where they are — literally — providing treatment in homes, workplaces, and community spaces. This model is designed to serve individuals with the most complex needs, helping reduce hospitalizations, enhance independence, and improve overall quality of life.

If you’ve ever wanted to make a deeper impact as a therapist, this is your opportunity. You’ll step into a pivotal leadership role, help shape the future of ACT services in northwest Kansas, and be a part of something truly transformative from the ground up. You won’t just work on a team — you’ll build and lead one, creating strong community connections and delivering life-changing care to those who need it most.

Join us in making meaningful, lasting change — not just for clients, but for the mental health system in rural Kansas. Apply today!

What You’ll Do:

  • Lead a multi-disciplinary ACT team to deliver non-traditional, community-based mental health treatment.
  • Coordinate services, manage referrals, and monitor staff availability, ensuring timely and effective treatment.
  • Provide clinical supervision to ACT staff, including crisis intervention and on-call duties.
  • Provide community-based therapy, including mental health and substance use treatment, to clients enrolled in the ACT program.
  • Conduct assessments, formulate Treatment Plans, and collaborate with clients, families, and the ACT team for the development of personalized treatment goals.
  • Lead daily ACT team meetings to maintain model fidelity and oversee clinical documentation.
  • Provide after-hours on-call crisis services as part of a rotation with other ACT team members.
  • Foster strong relationships with community partners and contribute to the growth of the ACT program.
  • Ensure compliance with all necessary regulations and Center policies.
